How do I fix blank item names and descriptions on my server?

It may be a stupid question with a simple answer, but I can't figure it out. I have blank-named items in my server. I don't know if it's normal or not, and i don't know how to fix it.
Also, if you know how to fix "NO MSG" windows, could you let me know how to fix those also?

Go to ur msg athena in ur conf. folder in ur server folder
